These terms describe how Ajax Harwood Clinic provides care, schedules appointments, handles payments, and communicates with patients. By booking an appointment or otherwise using our services, you agree to these terms.
About Ajax Harwood Clinic
Ajax Harwood Clinic operates as a sole proprietorship under Dr. Carlos Yu, a family physician licensed to practice in Ontario. The clinic provides family medicine, travel medicine, and group-based mental health services. Our office is located at 88 Harwood Avenue South, Ajax, Ontario.
Booking and appointments
Appointments may be booked online through our website or by phone. When you book an appointment:
- You will be asked to provide your name, contact information, and (for some services) a credit card on file
- You will receive a confirmation email with your appointment details and a reference number
- For some services, you will receive a short questionnaire to complete before your visit
Please complete any pre-visit questionnaire promptly. Your responses help us prepare and make the most of your appointment time.
Payment and cancellation
Payment terms vary by appointment type:
- Travel medicine consultations are uninsured services. Your card is authorized for the consultation fee at the time of booking and charged when you check in for your appointment.
- New patient appointments and referral consultations are typically covered by OHIP. A card is saved on file at booking but is not charged when you attend.
For all appointment types:
- If you cancel 48 or more hours before your appointment, there is no charge
- If you cancel within 48 hours, or do not attend without notice, a no-show fee may be charged to the card on file
- Vaccines, medications, and supplies are not covered by OHIP and are billed separately
Specific fee amounts are shown to you at the time of booking. We will confirm any uninsured fees with you before your visit.
How we communicate with you
We use email and our online forms to communicate with patients about appointments, follow-up information, and routine clinical matters. When you contact us through these channels:
- We aim to respond within 2 business days for non-urgent matters
- Email and online forms are not monitored continuously and are not appropriate for urgent or time-sensitive concerns
- Please do not use email to communicate symptoms requiring same-day attention
For urgent matters during clinic hours, please call (905) 683-0690.
For after-hours questions about your care, please call Telehealth Ontario at 811 or visit your nearest urgent care clinic.
For medical emergencies — including chest pain, severe bleeding, difficulty breathing, signs of stroke, suicidal thoughts, or any life-threatening situation — call 911 immediately or go to your nearest emergency room.
For mental health crisis support:

Use of clinic services
Our services are intended for patients seeking respectful, collaborative medical care. We ask that all patients:
- Provide accurate and complete information about their health
- Treat staff, clinicians, and other patients with courtesy
- Attend appointments on time, or cancel with reasonable notice
- Follow agreed-upon treatment plans, or discuss changes openly with us
The clinic reserves the right to refuse service for behaviour that is abusive, threatening, or compromises the safety of staff, other patients, or the physician.
Confidentiality and privacy
We protect the personal health information you share with us in accordance with the Ontario Personal Health Information Protection Act (PHIPA). Details about what information we collect, how we use it, and your rights are set out in our Privacy Policy.
Limitations
The information provided through our website, intake forms, and email communications is for general informational and clinical-coordination purposes. It is not a substitute for an in-person clinical assessment. Treatment recommendations, prescriptions, and clinical decisions are made during appointments, not through our website or contact forms.
Ajax Harwood Clinic is not a 24-hour service. We do not provide on-call coverage outside our published hours.
Ending the patient–physician relationship
In rare circumstances, the patient–physician relationship may need to end. This can happen for a number of reasons, including:
- The patient relocates outside the practice’s care area
- The patient and physician are unable to maintain a productive working relationship
- Repeated missed appointments without communication
- Behaviour that compromises the safety of staff, other patients, or the physician
If the practice needs to end the relationship, we follow the College of Physicians and Surgeons of Ontario (CPSO) guidelines: we provide reasonable written notice, continue urgent care during the transition period, and assist with finding alternative care where possible.
Patients are always free to seek care elsewhere at any time.
Changes to these terms
We may update these terms from time to time. The most recent version will always be posted at this URL with an updated “last updated” date. Significant changes will be communicated to active patients where appropriate.
